GALVA Union Pacific Railroads failure to conduct a risk assessment of a new light installation contributed to the probable cause for the collision of two UP freight trains in 2014 near Galva.
The National Transportation Safety Board released in December an investigation report into the Sept. 25, 2014, accident.
No crew members on either train suffered serious injury, but about 200 gallons of diesel fuel leaked from a tank and portions of the track and rail siding had to be replaced.
